Modified Sweat Chloride Test

Normally, Sodium Chloride ions are reabsorbed from the sweat glands into the body. In cystic fibrosis the Mutation of the CFTR gene Results in reduced Chloride reabsorption due to failure of chloride channel. Hence assessment of the Chloride concentration in sweat using pilocarpine i.e. the SWEAT CHLORIDE TEST remains the GOLD STANDARD INVESTIGATION to confirm the CF.

Harrisons 21st Edition suggests Correlating the test values with genetic testing and Clinical manifestations. This helps enhance the probability of diagnosing the CF and ensures there is no increase increase in the rate of false Positive in the diagnosis.

1: Low ( Changed from <40 to <30mmol/L) : Rule out the CF

2: Intermediate ( Changed from 40-59 to 30-59mmol/L) : Should Correlate with Genetic test results and clinical manifestations to diagnose CF

3: High: (>60mmol/L) : Diagnostic of CF
